New Patient Registration
Welcome to the Beacon Surgery
Thank you for choosing to register at the Beacon Surgery, we aim to provide you with a first class Primary Care Service.
Full details of the services we provide and other helpful information is included in our Practice booklet and on this web site, please take the time to familiarise yourself with the way we have organised the practice. In particular the sections on Appointments, Foreign Travel and Repeat Prescriptions, this will save you time and help you to get the most from our services.
Registration Process
Following patient feedback we have amended the registration process so that in most cases patients do not have to attend for a new patient check. However this does mean that all the information requested must be supplied on the forms which can all be downloaded from the pre-registration tab.
In order to register please;
- Complete the Health Authority form
- Complete the Ethnic Monitoring form
- Complete the Health Questionaire form
- Hand the forms to the receptionist together with
- Original documentation showing Photo ID (Passport, driving licence, ID card)
- Evidence of address (council tax bill, paid utility bill etc)
- a urine sample labelled with your name and date of birth
- a BP reading from the machine in the waiting room if you are over 18. Please ask if you require assistance
- your medical card – if available.
Please note that we will not be able to register you as a patient unless all the information requested on the forms is provided.
In our experience investing a small amount of time now on your health will ensure that we are better able to meet your future needs.
